Policy Servicing

Revival Scheme
- The policy lapses if premiums are not paid within the allotted grace period (refer Policy Terms & Conditions)
- A revival is a NOVATIO i.e. fresh contract thus, the Corporation is entitled to impose fresh terms and conditions where applicable
- Revival may be effected under any of the four schemes given below, whichever is applicable:
- Survival Benefit (SB)-cum-Revival
The SB Claim may be processed to deduct Arrears of Premium with Late fee, Loan and Interest on Loan.
Requirements:
- Discharge Form for Claim
- Original Policy Document
- Health Declaration form
- Medical Form (if required)
- Copy of Bank Statement
- Copy of valid photo ID - Loan-cum-Revival
A loan is calculated assuming the premium status is up-to-date and then processed to deduct the Arrears of Premium and Late Fee.
Requirements:
- Loan Application Form
- Original Policy Document
- Health Declaration Form
- Medical Form (if required)
- Copy of bank statement
- Copy of valid photo ID - Ordinary Revival
The full Arrears of Premium and Late Fee is remitted directly for the revival of policy.
Requirements:
- Full premium plus late fee as applicable
- Request letter for revival
- Health Declaration Form
- Medical Form (if required) - Special Revival
Under this scheme, the Date of Commencement of policy is shifted forward to reduce the amount of Arrears of Premium and Late Fee provided:- Last due date of premium (or First Unpaid Premium) is more than six months but within 3 years from the current date
- Number of premiums already paid does not exceed 3 years
