Loss of Policy Document

In the case of lost/misplaced policy document, an application for issue of special policy in lieu of the lost/misplace document may be filed as per Section 146 of the Insurance Act 1998
Requirements:
- Lost Policy Application Form
- Copy of Joint ID Card/TIN Letter
- $22.50 application fee for standard and $23.50 if accompanied with loan application
